Cure of obesity or overweight in combination with a bland hypo-calorie diet.

4. Action
The medication prevents the body’s uptake of some quantity of fat contained in feedings. Xenical is used for the treatment of obesity, in particular, for decreasing and retaining of body mass in combination with a low-calorie diet. Xenical may also be used in the cases not mentioned in this review article.
The performance of Orlistat is in the increase of fat concentration in fecal masses just after 24-48 hours after its intake. After the medication is cancelled in 48-72 hours the fat contains in stool usually returns to the level it has been before the beginning of the therapy.

5. Posology and Administration
Xenical should be taken in full conformity with the instruction or your doctor’s directions. If you do not understand the instruction ask your pharmacist, medical nurse, or attending doctor for aid.
Each tablet should be rinsed with a full glass of water.
You should take one capsule of 120 mg of Xenical with every main meal (during meal or not later than one hour after meal). If a meal is missed or if a meal does not contain fat you also can miss a Xenical intake.
If you increase the dose of Xenical more than it has been recommended it will not enhance the drug’s effect.
If you take Xenical with a very high fat content meal, a risk of the emergence of side effects and digestive upsets increases.

6. Counterindications
In no case take Xenical without first requesting your doctor in order to clear up the following cases:
- chronical syndrome of malabsorption (low absorption)
- gall bladder diseases.
- pregnancy,
- cholestasis (biliary engorgement).
Xenical is counterindicated to the people suffering from the above mentioned diseases and states.
Xenical is also counterindicated while breastfeeding, under 18 years of age and in hypersensitivity to Xenical.

7. Should Be Taken with Precautions, if…
Do not use Xenical before previous consultations with your doctor if you:
- suffer from renal lithiasis,
- suffer from diabetes,
- suffer from anorexia or bulimia,
- take any other diet pills.

8. Interface with Other Medications
In Xenical therapy a reduction of absorption of vitamins A, D, Е, К and beta-carotene is possible that is why it is recommended to take multivitamins together with Xenical.
If multivitamins are ordered it is better to take them not less than 2 hours after Orlistat intake or before sleep.
There have not been noticed interfaces with digoxin, meta-morphine, phenytoin, warfarin, oral contraceptives, nifedipine, statins and alcohol.
Body mass dropping due to Xenical therapy may improve a compensation of metabolic conversion in diabetes patients; as a result the request of a dose of oral sugar-reductive drugs may be reduced.

9. Overdosing
There are no described cases of overdosing of Orlistat. In clinical studies of persons with a normal weight and obesity patients the intake of single doses of 800 mg or multifold intake of the drug 400 mg 3 times a day during 15 days is not accompanied with the emergence of appreciable unwanted events. Moreover, obesity patients have an experience of Orlistat use 240 mg 3 times a day during 6 months.

In case of a frank overdosage of Orlistat it is recommended to watch the patient during 24 hours.

10. Side Effects
- oily discharge from straight intestine, flatus;
- acceleration of urges to defecate;
- pains or discomfort in stomach, liquid stool;
- allergic reaction.
As a rule the above mentioned side effects are low-grade and transient. They emerged on early phases of therapy (in the first 3 months), notably that most of the patients had not more than one event of such effect. Low-fat diet reduces the probability of intestinal tract side effects.
Moreover, on the part of intestinal tract the following unwanted events as pains or discomfort in stomach, meteorism, liquid stool, pulpy stool, pains or discomfort in straight intestine, cavities of teeth and impairment of gums may be observed.

11. Pregnancy and Lactation
Due to the absence of clinical data Xenical should not be administered to pregnant women.
There has not been studied the excretion of Orlistat with breast milk that is why it should not be taken during breastfeeding.
