3. Indications for Use.

It is used for the treatment and prevention of depressions of various origins, anxiety disorder and social phobias.

4. Action

It has a double action of performance both to depression and anxiety. Also it is efficient in the prevention of depressive positions.

5. Posology and Administration

Mild and moderate depression: 37,5 mg twice a day (after two weeks 75 mg twice a day, if needed).
Severe depression: 75 mg twice a day (after that a daily dose should be increased by 75 mg every 3-4 days).
The maximal dose is 375 mg/daily.
Special instruction:
When the drug is cancelled, as in the cases of treatment with other antidepressants, an abrupt stop of Venlafaxine therapy – especially after high doses of the drug -  may provoke cancellation symptoms, that is why it is recommended before the cancellation of the drug to reduce doses gradually.
Use in elderly patients:
the advanced age of a patient by itself does not require changing of doses, but caution is needed in the treatment of elderly patients, for instance, in connection with the possibility of disorders of kidneys functions. The least effective dose should be taken. If a dose of Venlafaxine is elevated a thorough medical screening should be carried out over the patient.

6. Counterindications

Preparations of Venlafaxine are counterindicated in the following cases: hypersensitivity to Venlafaxine or other components of the drug. Severe disorders of the functions of kidneys or liver, childhood and adolescence up to18 years of age (it has not been proved the security and efficiency of the drug for this age group). Determined or supposed pregnancy, lactation period (there are not enough date of controlled studies).

Most side effects depend on a dose of Venlafaxine. General symptoms: weakness, fatigability, anorexia, constipation, nausea, vomiting, dry mouth; sometimes – sleep tooth-grinding. Change of taste; change of liver functions, hepatitis, elevation cholesterin level, decrease of body mass; sometimes, increase of body mass.
Elevation of arterial tension, dilations of blood vessels (fluxions); faintness, tachycardia, disorders of ejaculation, disorders of erection, disorders of orgasm, difficulty in orgasm in women, difficulty in the beginning of urination; sometimes decrease of libido, excessive loss of blood in menstruations, retention of urination, disorders of vision; sometimes – disorders of taste, dermatorrhagia and effusion of blood to mucous membranes. Sometimes hypersensitivity to ultraviolet light.
After an abrupt cancellation of Venlafaxine or after the decrease of a dose there may occur fatigability, drowsiness, headache, nausea, vomiting, anorexia, dry mouth, dizziness, diarrhea, insomnia, anxiety, nervous irritability, disorientation, hypomania, paresthesia, sweatiness. Usually these symptoms are of low-grade and pass without cure.

11. Pregnancy and Lactation

There have not been done adequate and strictly controllable studies on the safety of the use of Venlafaxine in pregnancy (the safety has not been proved). The use during the pregnancy (or supposed pregnancy) is counterindicated; the use is possible only in the case of extreme necessity. Women of child-producing age should be warn about this before the start of the treatment, reliable method of contraception should be applied in the period of Venlafaxine treatment and they should inform the doctor immediately if in case pregnancy or if pregnancy is planned.
If it is necessary to take Venlafaxine during lactation breastfeeding should be stopped.
If the drug has been used immediately before child-birth, it is necessary to take into account a possibility of the progression of the symptoms of the drug cancellation in the newly-born.
